Forminator File Upload Stopped Working

We launched the updated version of the Player Registration form for 2025 a few weeks ago. The form includes a field to upload a headshot image file. Everything was going fine, we took over 30 submission with this year’s form since launch without issue. Then, out of nowhere, today, players trying to register couldn’t upload a file.
Clicking the “Choose a file” link does nothing. Like, nothing happens when the link is clicked . . . as if the code is incorrect. When using the drag-and-drop feature the image dropped into the box opens, full-screen, in the browser window — but doesn’t get loaded into the form.
We recreated the problem on several devices, both mobile and desktop, using different browsers. It’s pervasive. We tried deleting the field and adding a new upload file field, but that didn’t fix it. We tried changing the settings from single file upload to multiple; that didn’t help either. We are using the latest version of Forminator.
Interestingly, it works when we “Preview” the form while in the Edit Form interface.
But, otherwise, we’re out of ideas. It’s very strange because once we got a few minor details fixed in the first few days since it was published, we haven’t touched the form at all before this problem arose earlier today. If anyone can help with an answer, we would appreciate it. Thank you.
