Forminator form not showing up

This form is for completing online purchases. It has worked fine for months and suddenly disappeared.

I deactivated all plugins except for Forminator, that did not fix it.

I went into the theme (Rosa) customization and added `.forminator-custom-form {
display: block !important;
}`

The form appeared, however the calculation fields were empty and the credit card field was not appearing at all.
