Okay, so I’ve been trying to figure out the problem to this, and I believe it is down to this plugin.

On the link provided, when you submit a form, it does get added to the DB, but the form doesn’t update nor is there any redirection; it is literally like the form failed.

However, I have tested the form using the shortcode alone, and it does work.

It literally seems to be the confirmation part which fails (perhaps the AJAX response is the issue? I cannot see any error in the console).

The page I need help with: [log in to see the link]

This plugin does not affect the frontend of the website or the submission of the Gravity Form. It only provides a custom field type for use in your backend field group selector.

The output of the form in the frontend of the website is part of your Theme, Plugin or custom code, which is not affected by this plugin.

Whenever I’ve used this plugin on a production site myself, I’ve simply pulled the form ID selected by using the ACF field, and used that to output the result of the regular shortcode. If the shortcode and its parameters are correctly used, then there is no difference between this method and the regular, hard-coded use of the shortcode.
