Forms Display Loading Dots

Hi WP team,

I’m using the latest version of WP & NF on a client site. I’m also using a custom theme they asked me to design. The Ninja Forms have the Conditional Logic Plugin. Other than that there isn’t anything special here but the forms do not render when you land on pages containing the forms. If you refresh the browser on chrome or iphones browser the form does show up after doing that more than a couple of times.

Is this a known issue? I recommend this product to my client and now they are thinking of going with another vendor. Any suggestions before we jump ship?
