Forms/fields to BD and show on web

Hello,

I would like to create a form where users can fill in some fields and upload files (audio and image) and also can put the coordinates of a location or a location and the possibility of adding more location points.

I need this data to be stored in a database and then be able to collect it to put it in an APP and also show it as events on the website.

Then, I would like to know how I can do this, if there is a plugin or how to recover fields from the database to display them on a page
