If you’re using the same form on different pages, you can use the {page_title} Smart Tag in the Email Subject field of your form notification (here’s a screenshot of an example) This will pull the page title information into the email subject when the form is submitted.

Currently we do not have free addons for file upload, and the file upload field is available with any of our paid licenses.

We don’t recommend using third-party addons for the file upload field as they may not work as expected, and we aren’t able to provide support for them.
