Forms getting duplicated

We have seen some sporadic reports of this but I have not been able to reproduce it.

The new donations list: https://mydomain.com/wp-admin/edit.php?post_type=give_forms&page=give-forms loads using React. It’s possible that the query is conflicting with another plugin or the theme itself.

Can you attach your system info found in Donations -> Tools -> System Info?

This will give us more context to try to replicate this issue and solve it.
