Forms not adding to Mailchimp list

    I’ve added a submit form to my bands website but it’s not submitting to the selected newsletter list I’ve added it to. I’ve added an image of the back end so you can see that the form is linked to the mailchimp account and has the list ‘Indigo Bay’ selected (the only list on the mailchimp account)
