Forms Not working

There are 2 forms on this site that aren’t working. This is the vehicle release form – it just errors with the message but no information as to why. I have post smtp configured and functioning. The contact form sends properly. This form and the work authorization forms are not working. The work auth form doesn’t even show an error.
I have deactivated all plugins and themes and tested with the same results.
