forms to database

  • Is there a plugin available that will allow me to create forms for users of my wordpress site to fill in information like lunch orders that can then be stored in a database that I can export reports from?

    Ideally users can both register with this plugin, and I can also create forms to gather information from them and then export reports..

    Any way you can point me in the right direction if this exists?
