Free Contact Form for Latte

I followed the instructions to get the free Contact Form add-on for my Latte website (not Pro), and it still isn’t working. At first it said there was a configuration error, but once I fixed it, the Contact Form is still not appearing on my website.

The Contact Form is enabled and everything, and there’s no reason that I can see for it not to work.

Would very much appreciate help!
