Front End Form Not Loading

Hi, I have the block [ameliastepbooking] on my booking page and when I go to the site nothing loads. I’ve cleared my cache and tried going to the page from an incogneto window however the page appears blank apart from the page title. I’m also getting this result with my event page. I went through the customize section in the plugin to make sure both my forms 1.0 and 2.0 were updated and published, I received the “success” message when doing this. I double checked employees and services to ensure everything was up to date there and not causing a block, employees show available, the hours and services are all correct. I also tried to switch to [ameliabooking]with no success. I tried to contact customer support through the wp amelia website and the page won’t load. Can I please get info on how to resolve this issue?
