    I think that plugin has everything i need but i cant find how i can create a custom customer form.

    There is a new shortcode now [wc_book_my_service] using this shortcode you can make your customers to

    – Select a manufacture
    – Select device related to manufacture
    – Select service
    – Add customer information

    And a job is started for that customer. Please use this shortcode and you will get what you need thanks.
