Gender and Occupation Drop Down

I am using the 2.6.6 version of this plugin. I have implemented the Gender and Occupation drop downs (see here) and it is not actually showing up when the lead submits a form. I know because I entered my own data for a submission, selected an occupation and gender, and then when the sales force team got the new lead, that info was missing.

What did I leave out? Here is what I have.
