Get Value from PHP Function and Form Value?

  • I would like to bump this topic please. I am looking for a similar solution where I want to have a checkbox called (send conformation). If the checkbox is selected then they user will receive the email associated with that form – if the checkbox if left empty, then the user will not receive the email.

    Any help will be appreciated ??
