Getting Started seriously simple custom theme

Want to develop a simple theme. Think header w/image, menu on side bar main content and footer. Want to do one thing at a time to see how it acts before moving on. Want to be able to place things where I want.
Did an online tutorial

It did show at all you need is a css page and an index.php page in order to install the theme which worked local and on host.
Then bought the book Digging Into Word Press 3.1 and went through it. Followed all of that, don’t have it memorized or anything close though. Did not see a “okay lets make a simple one for some hands on” in it.
Tried to reverse engineer a few themes to include twentyten. Can not find where you would rest size of the header image etc. With all of its built in functionality, it is no doubt way more complicated than I need for a beginner sample. Know basic php, css, html, JavaScript, how to work with a framework etc, but could use either a simple seriously simple, theme to dissect or a tutorial that works on how to do it. Can someone point me in the right direction?
