Gift Card Recipient Details Box missing

HI, I added this plug in to my site a couple of days ago, and all was working fine. I have however just noticed that the box where you add the persons details you want to the Gift Card to go to on the product page has disappeared. I have tried deactivating and deleting and re-adding the plug in but it is still not there!
