Goal Completion Location showing as Entrance sometimes

So we have the plugin installed and are using Event for the Goal Type. We noticed that is tracking the total number of Goal Conversions correctly, but it often shows “(entrance)” as the value for Goal Completion Location instead of the actual Page URL. For example, on this goal only lives on one page of the site and in November I confirmed there were 51 Entries. https://www.screencast.com/t/b7jHbf8H7o But why are 19 showing as “entrance”?
