goes to blank page in 2.6.2

I just installed WP-CRM per https://www.slipfire.com/wp-crm-58.htm

Followed the instructions to a tee, and doing it on a fresh install. My site is https://www.seeiftheypay.com/

When I click on “Add Contact,” it goes to a blank page.

Even when I’m in admin, if I click “View this page” from my “Add contact” page, it goes to a blank page.

I verified that the slug is correct, andI’ve got the form set to dump into the page, but nothing. I tried adding a few words to the page, and when I clicked “View this Page” in admin, it still went to a blank page.

Not sure if this is an issue with TDO mini forms, or with my install.

Any ideas? I’m considering doing a new install with 2.5.1, but would prefer to keep it at 2.6.2
