From the 1st view I think it’s possible that on test form pages there is some problem (because on result page adsens is ok).

But you also have problems and client-side javascript errors. So I can’t determine where is the problem.

Try the same as another user did in the link I sent you.

Regarding other client-side errors (not only in test pages) — try to hire someone to fix them all. This could be anyone who knows wordpress and javascript.
