Hello,

It seems that the sheet columns are not mapped with form fields. Please create the table titles in the sheet. Like First Name, Last Name, Email, etc. Titles must be in the first row of the sheet. Now create new integration again, when you’ll select the spreadsheet and worksheet – the titles will be fetched and you’ve to map which form field you want to send to which title.
