Gravity Forms Persistence Plugin – How to Get It To Work?

I downloaded the Persistence plugin for Gravity Forms. I have 2 questions:

1. I installed Persistence, went in to an existing form I’m working on and checked the box “Enable Form Persistence” and updated form settings. When I go back in to make sure, the box is no longer checked. How do I get this to stay checked?

2. When I run the form, no log in or registration prompt comes up. This is required for Persistence to save the form people are working on, but I see no documentation in the plugin covering this. What am I doing wrong? People do not log in to my site.
