Great Contact and Document Upload Plugin!

  • Captain Form is a great plugin! I needed to be able to have students upload documents with their application and this form did everything I needed in one plugin! The ability to design the form the way that I wanted was so easy with this plugin.
    I couldn’t find one form field to change so I contacted support and they were very helpful.
    I only have the free version but I’m probably going to upgrade.
    This is an awesome plugin! Highly recommend it!
