GWebPro Store Locator errors on lines 215, 513 ad 517 on the front-end

I have come across this plugin and it seems to have exactly what I need.

However, there were some issues with the installation. I am running:
– Elegant themes Divo Theme
– WordPress 3.9.1
– GWeb Pro Store Locator 1.0.3

I have installed the plugin using the shortcode on the page:
[GwebproStoreLocator]

Here is what the published page looks like:

As you can see – there is an error notice on the search box for search by keyword:

There is another error notice, in the search results part of the page:
