    my sites were recently hit with a spam hack.

    paid someone to clean it, seems to be good, but I’m still getting spikes of traffic with searching for links like abc.com/?Vintage-Gitars/

    As I understand it, if the site has been cleaned properly, it should display a 404 right? It isn’t, and the browser shows my main page. Does that mean it wasn’t cleaned properly?

    Can you provide a URL to your site?

    The traffic spikes may simply be that the search engines indexed your spammed pages before they were removed.

    I understand the spike, my concern is more about my own site. Why is it still displaying the main page even though the above link shouldn’t exist?

    The question mark is used to pass parameters to a page. So, yes, this will go to your home page.
