Header is displayed on every page

    I’m very impressed by your plugin and its simplicity. Even though I encountered two bugs:

    The header-image is displayed on every page, even if this option is disabled. This happens in the preview PDF and in the one that is send.

    Under the header-image on all following pages (not on the first page), half of the page is empty. This only happens in the PDF that is send, not in the previewed PDF.

    I have to add that I dont use <addpage> because I need to have automatic pagebreak.
