help finding a plugin

Hi all,

I have been looking for a plugin that allows me a add a front end option so that customers that are not logged in can setup an email reminder for 1 year in the future.

Therefore, they just add their details, (Name, Phone number, Email & Car Reg) and choose a date and they get an email reminder at certain intervals (1 month, 2 weeks, 1 week, 1 day) before the due date.

I have found plugin called, email reminder, but it has limited function and no front end for customers.

Any ideas ?
