Help finding a plugin or building one

  • I am fairly new to WordPress and I have been searching high and low for an existing plugin to do this task but havent found it. So i need help finding it or help with building one….but heres what i need done..

    I want a user to be able to input a 4 digit number(that was already given to them) into a text field box and hit submit and that number will be found in a predefined list of numbers (defined by me, and each number represents a specific URL), and the user will be taken to that link.

    Right now this is my page, https://www.yevohawaii.com/join-the-hawaii-team but i just have each link listed out on the page currently. My goal is to have my site visitors able to hit those links but not see the entire list displayed. THANK YOU IN ADVANCE FOR ANY HELP!!!!
