Help with Payment form

Hello,
I need a payment form plugin to replace the one I am currently using on my website (eForm – wp form builder). I need a form like this one:

but with the option to have different “payment fees” depending on the selected payment method. To clarify, if a user selects offline payment method payment fee should be $0, but if for example paypal is selected, the fee should be lets say 2%.
Does anyone know if it is possible to create something like this in WooCommerce?
