Help with programming a plugin

hey all
i am not a great programmer – i made a stab at making a little plugin but didnt turn out. wondering if anyone out there wants to help ? or knows how this can be done.

I need to replicate it for this years season. I wanted to do it using WP – by having some sort of admin form to:

Select Team
Select Practice or Game
Enter Date and then submit

then have a page that will display results ( all teams at once or team by team, whichever is easier)

if anyone has any ideas or suggestions or would like to help i WOuld REALLY appreciate it
