Hidden form that shows up once entered?

Not even sure if this is possible with contact form 7, but im not much of a db guy, and self taught html coder.
Im looking to re-create the following form https://daviddunhaminsurance.com/
If you notice once you enter your name and tab over, then the rest of the form populates.
Anyone explain to me how to do this?
