Hello, the reason the default title is shown is similar to the previous forum post you linked to: the original image is not being used in the link.

    Example:

    • Original Image: DSCF0009.jpg
    • Default Title: DSCF0009
    • Linked Image: DSCF0009-scaled.jpg

As you can see, the default title generated by WordPress matches the original image’s file name. The linked image has a different file name that does not match the default title, so it is not filtered.

Linking to the original image will allow the default title to be filtered.

Are you using a plugin that generates “scaled” versions of uploaded images and updates links? If so, then disabling that plugin so that the original link is unchanged will allow the default title to be filtered.
