Highly recommended

Out of all forms apps that I tried (literally dozens), this one by far had the most features and worked best for me. I modified it slightly from a registration solution, to a change management solution for my organization. Also love the ability to schedule analytics reports. Support is very responsive. Highly recommend this if you need a fairly robust forms builder.
