How can I add more questions on registration format

    I really LOVE this pluggin! But a few troubles I am facing.

    I was trying to add more questions on registration format.
    I wanted to make it like

    Name
    Address
    City
    State
    Zip-code
    E-mail
    Phone number
    Comments

    Whenever I tried to add more information, there’s error.
    Could you possibly write me an code so I could copy and paste it?

    Also, How can I change the format so that the top page will have the date and the starting and ending time? It shows only as Apr 13 2013 – 10:00. So that it will have Apr 13 2013 – 10:00 to 3:00
