How do I add GA info to a form?

Hi,
I would like to track the user who filled out our forms. I’ve followed the instructions here: https://contactform7.com/tracking-form-submissions-with-google-analytics/. What should I do next?
I would like to have the following info appear in our forms:
The Google Analytics cookies associated with this person at the time the form was submitted are:
__utma: sample data
__utmb: sample data
__utmc: sample data
__utmz: 99158337.1449280145.1.1.utmcsr=google|utmccn=(organic)|utmcmd=organic|utmctr=(not provided)
