How do I add the rates to the frontend?

Similar to this topic: https://www.ads-software.com/support/topic/requesting-help-document/ I cannot figure out how to actually get the new rates on the frontend. I have set up an extended flat rate, but it is not being used in the cart.
Some screenshots showing the steps to adding the new rates to WooCommerce and the frontend would be very helpful.
