How do I display the same form on the order creation page?

We believe that you would like to include custom fields created using our plugin while placing a new order through WordPress.

If so, in order to include custom fields while creating a new order through WordPress. The custom fields will be available in the section “Custom Fields“. You can select the custom fields created using our plugin from the select box inside the “Add New Custom Field:” and provide value for that field there. If your custom field is not available inside the select box, click on the “Enter new” link and enter the custom field’s field name and provide the value.
