How do I get the name, email and website textboxes on comments ?

No – it’s not been removed. Try using <?php comments_template();?> at the appropriate point in single.php.

I haven’t tried that.

I haven’t altered the default theme and its not showing there too.
Surely it has to be a setting somewhere ?

I do have the actual comments box and the submit button.
