How do I populate an image with user initial input?

Hi All,

I have a page on my website that will require visitors to input their name and a 10-digit code via a simple two-field form along with a submit button.
I want to use this information (ie, name and code) to populate a printable card that will appear after the visitor clicks the submit button.
Can someone please tell me how to go about doing this? I’m not very techy, however I can create the first part using html. I just don’t know how to make the second part work. Thanks in advance for any help provided.
