How do I Pre-Fill Fields on an embeded form using fields from UM

Hi, I have a form (Not a UM Form) that I would like to embed onto a page within my site, whereby I want fields to be pre-filled based on URL parameters to pre-fill the users user_email user_ID how would I do this please?
