How do you show a form for visitors to submit a testimonial?

I see in the screenshots a short-code to show a form for visitors that allows them to submit a testimonial. But, in the plugin, it says that this shortcode [wpmtst-form] (along with all Original shortcodes) is being deprecated soon. So how are we supposed to show a form for visitors??
