How To?

I am currently operating a website for Car Insurance California I have been trying to convert to word press. My problem is the main function of the site is a contact information capture form. So far, I have not been able to find a plug in or way to replicate the way my current form works. Can anyone make some suggestions or provide advice on how to replicate my current set up?
