How to add Contact form to page

I created a Feedback menu option and I would like that to open to a Contact Form to gather customer feedback. I’ve tried adding a Contact plugin, but it just makes my sidebar disappear.

Is there a way to just make this post a simple contact form? (Name, Email, Feedback / comments)
