How to add pre set field name as additional field

  • Hi Team,

    I have below query

    1. I want to use three column in a filed as we can see Name that uses three different level( i.e First Name, Middle Name, Last Name)

    For Above Requirement If I press “Add New Field” It should have three column with three different columns.

    It it possible to create add new field option on form it self, So that if a user want to add more filed as per his request he can do its own.

    No, sorry this form does not have conditional fields features. Those kind of features are too complex for a free plugin.
