How To Attach Form Submissions To Custom Type

Hi There,

I’ve created a custom content type called “Plants”, and each “plant” I’d like to attach gravity form submissions. For example, I have a daily maintenance form. I’d like all forms submitted for a particular plant to be listed on that Plant’s page.

So you can see a record of form submissions on the plant page. I don’t need to load the data from the gravity forms, just the title and link to the data.

Can someone point me in the direction of how i might do this?
