how to collecting customers information?

So when customers try to make an appointment.
we want their first, last name, address, phone number all to get collected. when I click on their appointment time I should see all of that. I don’t want to have to chase the info down in other parts of the plugin.
We have tried to reach out through your website for help. several times. my 14 day trial is almost up and we haven’t heard anything back.
We said we would give you access so you can get it set up the way you advertise that it works. we are not seeing that and finding lot of bugs.
