How to disappear the form after submitting the it?

    Hey,

    I have build a radio type form to collect survey types answers from my visitors. Visitors can choose and the answer and submit it. All are OK but the original form stays even after submitting the form. It is showing an confirmation success message though below the form. But, I just want to show only the confirmation success message after submitting the form not the original form?

    Is there any to hide/remove the original form to visitors when they submit it?
