How to embed a MS-word Form

  • Hi,
    I am looking for a plugin or a way to embed a MS-word Form that users can complete and send it and answers back to me.
    It is a volunteership Form for an organization, so basically, the form contains all sorts of questions and answers, like multiple choices etc.

    Any idea?
