How to embed login box in a page

I’ve been trying to embed a login box within a page, because I want to style it to match my theme. I’ve tried adding the php from the codex and that doesn’t work (just displays nothing at all) and then tried installing the php in posts plugin and doing it again, but still nowt! How can I do this?
