how to find the Field ID?

You can change the field value by using shortcode created for field.

In your case it would be [smfield print=name]

We use the email address provided by WordPress as From address.
You can change your From address by using one of the following plugins.

and what abaut the “Single Line Text” field? what code to use?

[smfield print='fieldLabel']

You just need to replace the fieldlabel with the label of created field.

If single line text field label is Question, replace fieldLabel with Question.
